2. This contribution is not made from the general treasury funds of a corporation.
3. I am a citizen of the United States or hold a permanent Resident Card (green card).
 

Please Note:
1. Ohio law only allows for personal contributions to political action committees like Preserve Lakewood School PAC.  Corporate contributions cannot be accepted.  Please contact us if you would like to contribute through a partnership or other unincorporated
entity.
 

2. The information requested on this form is required by Ohio law. We are required to file this information with the Ohio Secretary of State and/or Cuyahoga County Board of Elections. It will not be distributed to any other agencies or used for marketing purposes outside of Preserve Lakewood Schools PAC.
 

3. Contributions to Preserve Lakewood Schools PAC are NOT tax deductible. Contributions are subject to the limitations and prohibitions of Ohio Election Law.
 

4. All donation transactions are final and not refundable. Contact info@preservelakewoodschools.org with special requests or circumstances.
 

5. Contributions to Preserve Lakewood Schools PAC can be made in the form of personal checks, money orders, cashier’s checks and through the online portal in our website, as well as certain in-kind contributions. If you prefer to contribute by a physical check or other means, please contact us at info@preservelakewoodschools.org.
